Sacroiliac Joint Pain: Symptoms, Causes, and Treatment Options
Lower back pain does not always begin in the spine. For some patients, discomfort comes from the sacroiliac joints, also known as the SI joints. These joints connect the sacrum at the base of the spine to the pelvis and help transfer weight between the upper body and the legs.
When an SI joint becomes irritated, inflamed, or moves abnormally, it may produce pain that resembles other back, hip, or nerve conditions. Because the symptoms can overlap, receiving an accurate diagnosis is an important first step toward finding the right treatment.
What Does Sacroiliac Joint Pain Feel Like?
SI joint pain is commonly felt in the lower back or buttock, often more noticeably on one side. It may also extend into the hip, groin, or back of the thigh. Unlike some forms of nerve pain, symptoms do not always travel below the knee.
Patients may notice increased discomfort while:
1. Standing up after sitting
2. Walking upstairs
3. Standing on one leg
4. Turning in bed
5. Sitting or standing for extended periods
6. Taking long strides while walking
7. Transitioning in and out of a car
The location of the pain alone is not enough to confirm that the SI joint is responsible. Disc problems, arthritis, muscle strain, hip disorders, and irritated spinal nerves may cause similar symptoms. A careful medical history and physical examination help distinguish between these potential pain generators.
What Causes SI Joint Pain?
The SI joints are supported by strong ligaments and surrounding muscles. Although they normally allow only a small amount of movement, either too much movement or too little movement can place stress on the joint and nearby tissues.
Potential contributing factors include:
1. Repetitive physical stress
2. A fall or other injury
3. Pregnancy related changes
4. Differences in leg length
5. Arthritis or inflammation
6. Previous lumbar spine surgery
7. Changes in walking mechanics
8. Weakness or imbalance in the muscles supporting the pelvis
Some patients develop symptoms gradually without a single identifiable injury. Activities involving repeated twisting, uneven weight distribution, or prolonged standing may aggravate the area.
How Is Sacroiliac Joint Pain Diagnosed?
Diagnosing SI joint pain may require several steps because routine imaging does not always identify whether the joint is actively producing symptoms.
During an examination, the physician may apply pressure or position the legs and pelvis in specific ways to reproduce the patient’s usual pain. A combination of multiple positive provocation tests may provide stronger evidence that the SI joint is involved than any single test alone.
X rays, magnetic resonance imaging, or other imaging studies may be ordered to evaluate the surrounding spine, hips, and pelvis or to rule out other causes of pain.
An image guided diagnostic injection may also be considered. During this procedure, a small amount of anesthetic is delivered into the SI joint. Meaningful temporary relief may help confirm that the joint is contributing to the patient’s symptoms.
Conservative Treatment for SI Joint Pain
Treatment is generally based on the severity of symptoms, the suspected cause, and how the pain affects daily activities.
Physical therapy is often an early part of treatment. A therapy program may focus on pelvic stability, core strength, posture, mobility, and flexibility. Manual therapy and targeted stretching may also be recommended when appropriate.
Additional conservative options may include activity modification, temporary use of a pelvic support belt, heat or cold therapy, and medications selected according to the patient’s medical history.
Treatment should not simply focus on avoiding movement. In many cases, gradually restoring strength and normal movement patterns is an important part of improving function.
Sacroiliac Joint Injections
When symptoms continue despite conservative treatment, an SI joint injection may be recommended for diagnostic or therapeutic purposes.
The procedure is commonly performed with fluoroscopic guidance. Fluoroscopy provides real time X ray imaging that helps the physician guide the needle accurately into the joint. Contrast material may be used to verify positioning before medication is administered.
Depending on the treatment plan, the injection may include a local anesthetic and corticosteroid. The anesthetic may provide temporary relief soon after the procedure, while the corticosteroid is intended to reduce inflammation over time.
Relief varies among patients. Some experience meaningful improvement, while others may have limited or temporary results. The patient’s response can also provide useful diagnostic information and help guide the next stage of care. Research supports the use of several conservative and interventional approaches, although no single treatment is appropriate for every patient.
What Happens After an SI Joint Injection?
Patients are typically monitored briefly after the procedure. Mild soreness at the injection site may occur and generally improves within several days.
Patients may be asked to track their symptoms carefully after the injection. It is helpful to note whether the pain improves, how long the improvement lasts, and which movements become easier.
Temporary numbness or weakness may occur because of the anesthetic. Patients should follow all transportation, activity, medication, and recovery instructions provided by their medical team.
Advanced Treatment Options
If pain returns after a successful diagnostic injection, additional treatments may be discussed. Depending on the diagnosis, these may include repeat injections, radiofrequency ablation of the nerves carrying pain signals from the region, regenerative medicine procedures, or surgical evaluation in carefully selected cases.
The goal is not simply to treat the location where pain is felt. Effective pain management requires identifying the structure responsible for the symptoms and selecting a treatment that fits the patient’s condition, health history, and functional goals.
