Radiofrequency Ablation vs Epidural Steroid Injection
Back pain treatments are not interchangeable. Two procedures may both be described as injections for back pain while targeting completely different structures and symptoms.
Radiofrequency ablation, commonly called RFA, is generally used for pain coming from the small facet joints of the spine. An epidural steroid injection is generally used when an irritated spinal nerve is contributing to pain that travels into an arm or leg.

The Main Difference Is the Source of Pain

Symptoms alone cannot confirm facet joint pain. Disc problems, muscle pain, sacroiliac joint dysfunction, and other conditions can create similar discomfort. A detailed examination is needed to determine which structure is most likely responsible.
What Does Nerve Root Pain Feel Like?
Nerve root pain often travels away from the spine.

How Does an Epidural Steroid Injection Work?
During an epidural steroid injection, medication is delivered into the epidural space surrounding the spinal nerves.
The corticosteroid is intended to reduce inflammation around an irritated nerve root. A local anesthetic may also be included depending on the procedure and physician’s plan.
Epidural injections may be performed through different approaches. A transforaminal injection targets the area where a particular nerve exits the spine. Which Procedure Works Faster?
The timing of relief varies.
An epidural injection may provide relief relatively soon when inflammation is a major contributor to nerve pain. Some patients experience meaningful improvement, while others notice only temporary or limited change.
RFA may involve a different recovery pattern. The treated area can remain sore or irritated before the full effect becomes clear. Because RFA changes how a nerve transmits pain, improvement may develop gradually rather than immediately.
Patients should not judge either procedure solely by how they feel during the first day or two. Their physician can explain the expected recovery based on the exact technique performed.
Which Procedure Lasts Longer?
Epidural steroid injections are generally intended to provide temporary control of nerve inflammation. Evidence supports short term benefit for selected patients with radiculopathy, but consistent long term relief has not been demonstrated for every condition.
RFA may provide relief for a longer period when facet joint pain has been carefully confirmed. The targeted nerve can eventually regain function, which means pain may return. Neither procedure should be described as a permanent cure.
Can Someone Receive Both Procedures?
Yes. A person can have more than one source of pain.
For example, a patient may have facet joint arthritis causing localized lower back pain and a herniated disc irritating a nerve that travels into the leg. RFA and an epidural injection would address different parts of that clinical picture.
They would not necessarily be performed at the same time. A physician may first determine which symptoms are most limiting and which pain generator is best supported by the examination and imaging.
How Does a Doctor Choose Between RFA and an Epidural?
The decision usually begins with several questions.
1. Where does the pain begin?
2. Does it remain in the back or travel into an arm or leg?
3. Are numbness, tingling, or weakness present?
4. Which movements make the pain worse?
5. What does the MRI show?
6. Has physical therapy or medication helped?
7. Did a diagnostic nerve block reduce the pain?
The procedure should match the pain generator rather than simply the location where the patient feels discomfort.
Is RFA or an Epidural Steroid Injection Better?
One is not universally better than the other.
RFA is usually more relevant when the pain is coming from the facet joints and diagnostic blocks support that conclusion. An epidural steroid injection is usually more relevant when inflammation around a spinal nerve is causing radiating pain.
