PRP vs. Cortisone Injections for Joint Pain: What’s the Difference?

Joint pain can make everyday activities, exercise, and even simple movements uncomfortable. When rest, physical therapy, and other conservative treatments are not providing enough relief, injections may be considered.


Two options patients commonly hear about are cortisone injections and platelet rich plasma, or PRP, injections. While both may be used for certain musculoskeletal conditions, they work in very different ways.


What Is a Cortisone Injection?


Cortisone is a type of corticosteroid medication that helps reduce inflammation. When injected directly into or around a painful joint, tendon, or other structure, it can decrease inflammation and provide temporary pain relief.


Cortisone injections are commonly used for conditions such as arthritis, bursitis, and other inflammatory causes of musculoskeletal pain.


One advantage of cortisone is that some patients experience relatively quick symptom relief. However, the goal of the injection is primarily to control inflammation and pain rather than stimulate the body's natural healing response.


The duration of relief varies depending on the condition and the individual patient.


What Is PRP?


PRP stands for platelet rich plasma. Unlike cortisone, PRP is created from a patient's own blood.


A small amount of blood is drawn and processed in a centrifuge to separate and concentrate the platelets. The resulting platelet rich plasma is then injected into the area being treated.


Platelets are best known for their role in blood clotting, but they also contain proteins and growth factors involved in the body's response to tissue injury.


The goal of PRP treatment is to place a concentrated amount of these platelets at the treatment site to support the body's natural healing processes.


PRP vs. Cortisone: How Are They Different?


The biggest difference is the purpose of the treatment.


Cortisone primarily works by reducing inflammation. For some patients, this can provide relatively rapid pain relief.


PRP takes a different approach. Rather than suppressing inflammation, PRP is intended to support the biological processes involved in tissue recovery.


Because of this difference, PRP results are usually not immediate. Patients may notice gradual improvement over the weeks or months following treatment.


Which Conditions May Be Treated With PRP?


PRP has been studied for several orthopedic conditions, including certain tendon injuries and knee osteoarthritis. Research continues to evaluate which conditions respond best and which patients are most likely to benefit.


PRP is not appropriate for every type of joint pain, and results vary based on the underlying diagnosis, severity of the injury, overall health, and treatment protocol.


Which Injection Is Right for You?


There is no single injection that is best for every patient.


Someone experiencing significant inflammation who needs short term symptom control may have different treatment goals than someone managing a chronic tendon injury or degenerative joint condition.


That is why identifying the actual source of the pain is important before deciding on an injection.
