PRP vs Cortisone for Joint Pain

When joint pain continues despite rest, physical therapy, medications, or activity changes, patients may begin considering an injection.


Two commonly discussed options are cortisone and platelet rich plasma, also known as PRP. Both can be used in orthopedic care, but they work differently and may be selected for different goals.


Cortisone is primarily intended to reduce inflammation. PRP uses components of the patient’s own blood and is intended to support biological processes involved in healing and tissue response.

What Is a Cortisone Injection?

PRP stands for platelet rich plasma.


A small amount of the patient’s blood is drawn and processed in a centrifuge. This separates the blood into components and allows a platelet concentrated preparation to be created.


Platelets contain signaling proteins and growth factors involved in the body’s response to injury. The prepared PRP is injected into the targeted joint, tendon, ligament, or soft tissue structure.


Because PRP comes from the patient’s own blood, it is considered an autologous treatment.


PRP preparations and treatment protocols can vary. The concentration of platelets, presence of white blood cells, number of injections, and condition being treated may all influence outcomes. This variation is one reason research findings are not identical across every study.


The Main Difference Between PRP and Cortisone


Cortisone suppresses inflammation.


PRP attempts to influence the biological environment of the injured or degenerative tissue.


This does not mean that inflammation is always harmful or that PRP can regenerate every damaged structure. Inflammation is part of the body’s normal response to injury, but excessive or prolonged inflammation can contribute to pain.


PRP should not be presented as a cure or as a guaranteed method of regrowing cartilage. Its purpose is to support a healing response and potentially improve pain and function in appropriately selected patients.


Which Injection Provides Faster Relief?


Cortisone often has the advantage when rapid reduction of inflammation is the main goal.


Some patients notice improvement within several days, although the timing and degree of relief vary. A temporary increase in discomfort can also occur after an injection.


PRP usually follows a more gradual timeline. Because the treatment is intended to stimulate a biological response, patients may initially experience soreness before noticing improvement. Changes may develop over several weeks rather than immediately.


A patient seeking quick relief before an important event may have different priorities from someone looking for a treatment that may provide more gradual improvement.


Which Injection Lasts Longer?


The answer depends on the diagnosis and the individual patient.


Cortisone can provide meaningful symptom relief, but the effect may be temporary. Some patients obtain substantial improvement, while others experience little change.


Research comparing PRP with corticosteroid injections for knee osteoarthritis has produced mixed but increasingly informative results. Several reviews suggest that PRP may provide stronger improvement at later follow up points, while cortisone may offer more noticeable relief early in recovery. Other analyses conclude that both can be effective and that evidence is not yet consistent enough to name one option as superior for every patient.


These differences reinforce the importance of personalized treatment rather than selecting an injection based only on popularity.


When Might Cortisone Be Considered?


Cortisone may be discussed when a patient has a painful inflammatory flare, significant swelling, or symptoms that are preventing participation in physical therapy or daily activity.


It may also be appropriate when a patient needs temporary symptom control and is not a candidate for PRP.


Medical history matters. Corticosteroids may temporarily affect blood sugar and can have other local or systemic effects. The physician should review the patient’s health conditions, medications, prior injections, and frequency of steroid exposure.


Repeated injections into the same structure should be considered carefully rather than treated as an unlimited solution.


When Might PRP Be Considered?


PRP may be discussed for certain cases of knee osteoarthritis, tendon injury, ligament injury, or persistent musculoskeletal pain.


Patients may consider PRP when conservative care has not provided enough improvement and they want to explore a regenerative approach before surgery.


Evidence for PRP is strongest for certain conditions and less established for others. A treatment that may be reasonable for mild or moderate knee osteoarthritis is not automatically appropriate for severe joint destruction, complete tendon rupture, infection, inflammatory arthritis, or pain coming from a different structure.

Is PRP Only for Athletes?


No.


PRP has become well known in sports medicine, but joint and tendon injuries affect people at every activity level.


A patient may want to return to running or competitive sports. Another may simply want to walk through New York City, climb stairs, lift a child, sleep comfortably, or return to work without persistent pain.


The treatment plan should be based on the condition and the patient’s functional goals, not whether the patient identifies as an athlete.

Certain anti inflammatory medications may interfere with the biological response sought after PRP, but medication decisions must be made with the treating physician. Patients should never stop a prescribed medication without medical guidance.

Ultrasound or fluoroscopy can help a physician visualize the joint and surrounding structures while guiding the needle toward the intended target. This is especially valuable when treating a specific tendon, ligament, joint compartment, or deeper anatomical structure.


Image guidance does not guarantee a successful result, but it helps ensure that the treatment is delivered where it is intended.

Is PRP Better Than Cortisone?
