Lumbar Radiculopathy: Symptoms, Causes, and Treatment

Lower back pain is common, but pain that travels into the leg may indicate irritation or compression of a spinal nerve.


This condition is known as lumbar radiculopathy. It is sometimes referred to as sciatica, particularly when symptoms involve the sciatic nerve distribution.


Lumbar radiculopathy can cause pain, numbness, tingling, or weakness. Symptoms may interfere with sitting, walking, sleeping, working, and exercising.


Treatment depends on the affected nerve, the underlying cause, the severity of symptoms, and whether muscle weakness or other neurological changes are present.


What Is Lumbar Radiculopathy?


The lumbar spine contains five vertebrae in the lower back. Spinal nerves exit through small openings between the vertebrae and travel into the hips and legs.


Lumbar radiculopathy occurs when one of these nerve roots becomes irritated, inflamed, or compressed.


The location of symptoms often corresponds with the affected nerve root. For example, one nerve may cause symptoms along the outside of the leg, while another may affect the front of the thigh or top of the foot.


Radiculopathy is different from pain that remains only in the lower back. A patient may have both axial back pain and radicular leg pain, but the treatments for each may differ.


What Causes Lumbar Radiculopathy?


Common causes include:

1. A herniated disc

2. Degenerative disc changes

3. Spinal stenosis

4. Bone spurs

5. Facet joint enlargement

6. Spondylolisthesis

7. Inflammation surrounding a nerve root

8. Less commonly, a cyst, fracture, infection, or mass


A herniated disc occurs when material from inside a spinal disc extends outward and irritates a nearby nerve.


Spinal stenosis occurs when the space available for the spinal nerves becomes narrowed. Symptoms from stenosis may become worse while standing or walking and improve when sitting or leaning forward.


A patient’s diagnosis should not be based only on imaging. Disc bulges, arthritis, and other spinal changes are common, even among people without pain. Imaging findings should be interpreted alongside the patient’s symptoms and physical examination.


Symptoms of Lumbar Radiculopathy


Symptoms may include:

1. Sharp or burning leg pain

2. Pain extending from the lower back into the buttock or leg

3. Numbness or tingling

4. Electric or shooting sensations

5. Muscle weakness

6. Reduced reflexes

7. Pain that becomes worse with coughing or sneezing

8. Difficulty standing, walking, or sitting comfortably


Symptoms can follow different patterns depending on the affected nerve.


Some patients have severe leg pain with relatively little back pain. Others experience a combination of lower back discomfort and leg symptoms.


When Is Lumbar Radiculopathy an Emergency?


Most cases do not require emergency treatment. However, certain symptoms require immediate medical evaluation.


Patients should seek urgent care for:

1. New loss of bladder or bowel control

2. Numbness around the inner thighs or genital region

3. Rapidly worsening leg weakness

4. Difficulty lifting the foot

5. Severe symptoms following a major injury

6. Back pain accompanied by fever or serious illness


These symptoms may indicate significant nerve compression, infection, fracture, or another condition that requires prompt treatment.


How Is Lumbar Radiculopathy Diagnosed?


The physician reviews the patient’s symptoms, including where the pain travels and whether numbness or weakness is present.


The examination may assess:

1. Muscle strength

2. Sensation

3. Reflexes

4. Walking pattern

5. Hip and spinal movement

6. Nerve tension signs

7. Positions that improve or worsen symptoms


Magnetic resonance imaging may be ordered when symptoms are severe, persistent, accompanied by neurological changes, or being evaluated for a procedure or surgery.


Electrodiagnostic testing may sometimes be used when the diagnosis is unclear or when symptoms could result from a peripheral nerve condition rather than a spinal nerve root.


Conservative Treatment Options


Many patients begin with non surgical treatment, provided there are no serious neurological warning signs.


Treatment may include:

1. Temporary activity modification

2. Physical therapy

3. Guided exercise

4. Anti inflammatory or pain medication when medically appropriate

5. Heat or cold therapy

6. Postural and ergonomic adjustments

7. Gradual return to normal activity


Extended bed rest is generally not the goal. A structured rehabilitation program may help restore mobility, improve trunk strength, and reduce positions that repeatedly irritate the nerve.


Medication recommendations should be individualized because anti inflammatory medications and other pain medicines are not appropriate for every patient.


What Is an Epidural Steroid Injection?


An epidural steroid injection delivers medication into the epidural space surrounding the spinal nerves.


The injection may include a corticosteroid intended to reduce inflammation and a local anesthetic that may provide temporary pain relief.


Epidural injections may be performed through different approaches, including:

1. Interlaminar

2. Transforaminal

3. Caudal


The appropriate approach depends on the suspected pain source, spinal anatomy, previous surgery, and the location of nerve irritation.


Fluoroscopy is commonly used to guide needle placement. Contrast material may be administered to confirm the flow of medication before the injection is completed.


How Effective Are Epidural Steroid Injections?


Epidural steroid injections do not repair a herniated disc or permanently reverse spinal stenosis. Their purpose is to reduce inflammation and pain surrounding an irritated nerve.


A 2025 review from the American Academy of Neurology found that epidural steroid injections may provide limited improvement in pain and disability for cervical and lumbar radiculopathy, primarily over the short term.


Other systematic reviews have found that epidural injections may reduce sciatica caused by lumbar disc herniation over short and medium periods. However, consistent long term improvements in disability or avoidance of surgery have not been demonstrated for every patient.


The potential benefit should therefore be considered alongside the patient’s diagnosis, symptoms, treatment history, and functional goals.


Why Might Temporary Relief Still Be Helpful?


Even when relief is temporary, reducing pain may help a patient:

1. Begin physical therapy

2. Improve walking tolerance

3. Sleep more comfortably

4. Return to work or daily activities

5. Reduce reliance on certain pain medications

6. Determine whether a specific nerve is contributing to symptoms


The goal may be to create an opportunity for rehabilitation rather than relying on the injection as a stand alone solution.


Potential Risks of Epidural Injections


Potential side effects and risks may include:

1. Temporary soreness

2. Headache

3. Bleeding

4. Infection

5. Elevated blood sugar

6. Facial flushing

7. Temporary sleep disturbance

8. Increased pain

9. Allergic reaction

10. Rare neurological injury


Patients should tell the physician about blood thinning medications, diabetes, infections, allergies, pregnancy, and previous reactions to contrast material or steroids.


The risks and benefits should be reviewed before treatment.
