Facet Joint Pain and Medial Branch Blocks

Chronic neck or lower back pain can develop from many different structures, including muscles, discs, nerves, and joints. One frequently overlooked source is the small joints located along the back of the spine, known as facet joints.


Facet joints help support the spine while allowing bending, rotation, and other controlled movements. When these joints become irritated or affected by arthritis, they may contribute to ongoing pain and stiffness.


Because facet joint pain can resemble other spinal conditions, diagnosis often requires more than an imaging study. A medial branch block may help determine whether the facet joints are responsible for a patient’s symptoms and whether radiofrequency ablation may be an appropriate treatment option.


What Are the Facet Joints?


Each spinal level contains two facet joints, with one located on either side of the vertebra. These joints connect adjacent vertebrae and help guide movement while preventing excessive rotation.


The facet joints are lined with cartilage and surrounded by a joint capsule. Small nerves called medial branch nerves carry pain signals from the facet joints to the brain.


Over time, cartilage inside the joints may wear down. The joints may become inflamed, enlarged, or stiff. Facet pain may also develop following an injury, repetitive stress, or changes in the way the spine distributes weight.


Facet joint pain is commonly associated with degenerative changes, but the presence of arthritis on an X ray or magnetic resonance imaging study does not automatically mean that the joint is causing pain. Many people have visible spinal changes without significant symptoms.


What Does Facet Joint Pain Feel Like?


Symptoms vary according to whether the affected joints are located in the neck, middle back, or lower back.


Lumbar facet joint pain may cause:

1. Aching pain in the lower back

2. Pain that extends into the buttocks or upper thighs

3. Stiffness after sitting or resting

4. Pain when leaning backward

5. Discomfort during twisting or rotation

6. Pain that becomes worse after prolonged standing


Cervical facet joint pain may cause:

1. Neck pain and stiffness

2. Pain across the shoulders

3. Pain near the shoulder blades

4. Headaches beginning near the base of the skull

5. Difficulty turning the head

6. Increased pain with neck extension or rotation


Facet pain usually does not follow the same pattern as pain caused by a compressed spinal nerve. However, symptoms may overlap, and patients can have more than one source of pain at the same time.


How Is Facet Joint Pain Diagnosed?


A physician begins by reviewing the location of the pain, activities that worsen it, previous injuries, medical history, and prior treatments.


The physical examination may assess spinal movement, tenderness, strength, reflexes, sensation, and signs of nerve irritation. Imaging may be ordered to evaluate arthritis, disc changes, spinal stenosis, fractures, or other possible causes.


However, physical examination findings and imaging cannot always confirm whether a particular facet joint is producing pain. Diagnostic medial branch blocks are often considered more useful than imaging alone when selecting patients for facet radiofrequency ablation.


What Is a Medial Branch Block?


A medial branch block is an image guided procedure in which a small amount of local anesthetic is placed near the medial branch nerves supplying a suspected painful facet joint.


The purpose is to temporarily interrupt pain signals from the joint. If the patient experiences meaningful relief while the anesthetic is active, this suggests that the targeted facet joint may be contributing to the symptoms.


The procedure is typically performed using fluoroscopy. Fluoroscopy provides real time X ray imaging, allowing the physician to guide the needle toward the appropriate nerves while avoiding nearby structures.


Contrast material may be used to confirm needle placement before the anesthetic is administered.


What Happens During the Procedure?


The patient is positioned according to the area being treated. The skin is cleaned, and a local anesthetic may be used to numb the injection site.


Using fluoroscopic guidance, the physician advances a thin needle toward each targeted medial branch nerve. A small amount of anesthetic is then delivered.


Several nerves may need to be blocked because each facet joint generally receives signals from more than one medial branch nerve.


The procedure is usually completed without a large incision. Patients are monitored briefly afterward before receiving recovery and activity instructions.


What Should Patients Track After a Medial Branch Block?


Patients may be asked to complete normal movements that usually cause discomfort while the local anesthetic is active.


It is helpful to record:

1. The percentage of pain relief

2. How quickly relief begins

3. How long relief lasts

4. Which movements become easier

5. Whether standing, sitting, or walking improves

6. Whether the patient can complete normal daily activities with less discomfort


The block is intended to provide diagnostic information. Relief may last only for the expected duration of the anesthetic.


A short period of relief does not necessarily mean that the procedure failed. The response is used to help determine whether the medial branch nerves are carrying pain signals from the suspected joints.


Why Are Repeat Diagnostic Blocks Sometimes Performed?


In some situations, a physician or insurance plan may require a second diagnostic block before approving radiofrequency ablation.


Repeat blocks may help reduce the chance of proceeding based on a false positive response. However, there is ongoing discussion within pain medicine regarding the ideal number of blocks and the percentage of relief that should be required.


Research suggests that careful patient selection can improve radiofrequency ablation outcomes, although very strict criteria may also exclude some patients who could benefit.


What Is Radiofrequency Ablation?


Radiofrequency ablation uses controlled thermal energy to interrupt pain signals carried by the medial branch nerves.


During the procedure, specialized needles are placed near the targeted nerves using fluoroscopic guidance. Testing may be performed to confirm positioning. Radiofrequency energy is then delivered through the needle.


The goal is to reduce pain from the facet joint while preserving the surrounding spinal structures.


The treated nerves may gradually regenerate, so relief is not necessarily permanent. The amount and duration of improvement vary among patients.


Evidence supports lumbar medial branch radiofrequency ablation as a treatment option for carefully selected patients with confirmed facet joint pain. Medial branch blocks are generally considered more predictive of treatment response than injections performed directly inside the facet joint.


Are Medial Branch Blocks Therapeutic?


Medial branch blocks are most often discussed as diagnostic procedures. Some patients may experience relief that lasts beyond the expected duration of the anesthetic.


A 2024 systematic review reported moderate to strong evidence supporting facet joint nerve blocks for managing spinal facet joint pain, although treatment plans and expected outcomes should remain individualized.


A temporary response may also allow a patient to participate more comfortably in physical therapy, strengthening, or other rehabilitation.


Potential Risks and Side Effects


Medial branch blocks are generally considered minimally invasive, but all procedures carry potential risks.


These may include:

1. Temporary soreness

2. Bruising

3. Bleeding

4. Infection

5. Allergic reaction

6. Temporary numbness

7. Increased pain

8. Injury to a nearby nerve or blood vessel


Patients should provide a complete medication list and inform the physician about blood thinning medications, allergies, pregnancy, infections, and previous reactions to contrast or anesthetic medications.
